Documents for obtaining a health insurance reimbursement

  • Passport
  • Birth Certificate
  • Document confirming the disability group
  • Examination prescription list
  • Examination results
  • Medical certificate issued on the form of a doctor or medical institution, signed by the attending physician.

The certificate must indicate:

- Name, surname of the insured person,

- Date of visit of the insured person,

- Completed and/or assigned laboratory and instrumental examinations,

- Final diagnosis,
- Prescribed treatment (list of medications, including dosages, number of days of taking the medicine).

  • Medical discharge report (extract from the medical history). The discharge report must be signed by the attending physician and sealed with the medical institution’s stamp. The discharge report must indicate:

- Name, Surname of the insured person,

- Date of visit and discharge of the insured person,

- Completed laboratory and instrumental examinations,

- Final diagnosis,

- Prescribed treatment (list of medications, including dosages, number of days of taking the medicine).

  • Prescription for medication (a diagnosis is required), which should include:

- List of medications, including dosages, number of days of taking the drug

  • Exchange card in dentistry:

- A certificate indicating the performed medical procedures

  • Financial document confirming the expense incurred (cashier’s receipt, invoice, pay-in slip)


An insurance claim application is to be submitted online here. When filling out the application, all the necessary documents should be attached to it. The documents must be presented to the insurer within 45 days from the date when expenses incur.

Coupons for the purchase of medications prescribed for an acute illness are valid for 5 days from the date of prescription.
